Applying For A Cannabis Growing License

I saw my doctor a few days ago, and we talked about cannabis and pain.
Gave him a copy of "It's Just a Plant" and he loved it.
He said no problem with prescribing cannabis for me, and if I brought in the application in, he would fill it out and sign it.

So I pulled down the application, of which there are 9 parts, each part a different license for some aspect of marijuana usage.
Form A, authorization to possess marihuana for medical purposes.
Form B,  B1, B2, various forms for the physician to fill out.
Form C, is to grow you're own medicine.
Form D, is to have someone grow your medicine for you.
Form E1, is to obtain government grown pot.
Form E2, is to obtain government marihuana seeds.
Form F, consent of property owner.
Then there are tables to tell the doctor how much you should smoke, anything over 5 grams a day is considered excessive.
Don't know if I want to do it though, seems like it is painting a pretty big bullseye on me.
First I give them my picture, I tell them I am growing cannabis, then I even give them the address , then they tell the police, and I have no idea what happens then.
What should I do?

I applied and I now can grow marijuana plants for my own medicinal purposes.

When a friend took me to his own holistic doctor for an evaluation, last month, I was given a doctors recommendation to use and grow medical marijuana. (Thanks Ede!) If I owned this property I'd start my own "Child's Garden of Grass" tomorrow. But I rent so I can't ....

Someday ...  getting it legally as medicine, at any of hundreds of dispensaries in my area, is a good first step.
